By submitting this form and clicking the option “I AGREE”, I, as the personal data subject, hereby consent to the processing of my personal data. which personal data I hereby voluntarily provide to Kewpie Indonesia by the method specified below in this consent. I acknowledge that the personal data which relates to my person represents the so-called personal data within the meaning of the Law No. 27 of 2022 of the Republic of Indonesia on Personal Data Protection. At the same time, I declare that I am (i) at least 18 years old and/or (ii) not a person with disabilities, who is under other guardianship and if not then I confirm that such consent has been expressed or given by the person who has the parental and/or guardianship responsibility for me.
The controller and the processor of my personal data is PT Kewpie Indonesia, a limited liability company established in Republic of Indonesia, with registered office at Kawasan Industri GIIC Blok. CC No. 5 Kota Deltamas Ds. Pasirranji, Kec. Cikarang Pusat, Kab. Bekasi Jawa Barat, Indonesia, 17530 (“Data Controller”).
I hereby understand and agree the Data Controller shall acquire and process my personal data in accordance with its Privacy Policy, as set out in following link https://kewpie.co.id/en/privacy-policy. I hereby declare that I have read and understood the Data Controller’s Privacy Policy for the purposes of processing my personal data.
This consent is an integral part of the Data Controller’s Privacy Policy.
I provide my personal data as a customer or prospective customer of Data Controller.
My personal data, which I voluntarily provide to the Data Controller are, including but not limited to, the following:

| (b) | the Data Controller’s product development, by obtaining audiences’ preference in seasoning’s taste. |
| (c) | the Data Controller's menu proposals, by obtaining audiences' preference in foods and beverages menu. |
| (d) | the Data Controller's insight on how to approach its audiences, by obtaining audiences’ thought on certain lifestyle (e.g., eating habit, healthy lifestyle, etc.,). |
| (e) | the Data Controller’s marketing purposes relating to offers of products and services; sending information about organized events, products, services and other activities (e.g. in the form of sent newsletters, telemarketing). |
| (f) | contacts for the purpose of market researches. |
| (g) | contacts for the purpose of holidays greetings and sending discount vouchers, gifts, prizes to competition winners; and/or |
| (h) | for giving feedback regarding my inquiries. |
The Data Controller shall process personal data which shall be adequate, relevant and limited to the extent necessary for the purpose of processing.
The Data Controller shall be entitled to transfer my personal data in particular to the following recipients and categories of recipients:
| (a) | the Data Controller’s vendors/suppliers/merchants. |
| (b) | the Data Controller’s employees, transport services providers, providers of marketing and advertising services. |
| (c) | government authorities within the scope of fulfilling the statutory obligations prescribed by relevant legal regulations; and/or |
| (d) | other parties in other contractual relationships with the Data Controller. |
The Data Controller shall be entitled to process the above personal data for a period of 5 years starting from the date of giving my consent. I acknowledge that prior to the expiration of this period the Data Controller may contact me with the purpose of renewing my consent and that without such renewal my personal data shall be, after the expiration of the above period, deleted forever.
I acknowledge that as the personal data subject, I have the right to demand from the data controller access to, the correction and/or deletion of and as the case may any limitation of processing of my personal data.

If I, as the personal data subject, am of the opinion that my personal data is processed in contradiction with legal regulations, I have the right to request remedial action from the Data Controller. If my request is found reasonable, the Data Controller shall immediately remedy the defective status.
I hereby give my consent to the processing of my personal data by the Data Controller for (i) the purposes and for the period of time specified above and (ii) in accordance with the Data Controller’s Privacy Policy and applicable laws and regulations.
I acknowledge that the Data Controller shall archive this consent for the purpose of fulfilling the Data Controller’s statutory obligation to be able to demonstrate that my consent to the processing has been given.
